Aria of Waukesha (formerly Virginia Health and Rehabilitation
Center)We are seeking qualified Certified Nursing Assistants in a
PART TIME capacity on the NOC shift. -CERTIFIED NURSING ASSISTANT
FUNCTION: -To provide routine daily nursing care and services that
support the care delivered to residents needing skilled nursing
care/complex medical care and/or rehabilitative care under the
direction and supervision of a registered nurse or a licensed
practical/vocation nurse.CERTIFIED NURSING ASSISTANT
QUALIFICATIONS: -The requirements listed below are representative
of the knowledge, skill and/or ability required.Minimum
Qualifications:
- Freedom from illegal use of drugs.
- Freedom from use of and effects of use of drugs and alcohol in
the workplace.
- Anyone found guilty by a court of law of abusing, neglecting or
mistreating individuals in a health care related setting are
ineligible for employment.
- Must be at least 18 years of age.Education and/or Experience:
-High school diploma or general education degree (GED), or
three-six months related experience and/or training or equivalent
combination of education and experience preferred.Certificates,
Licenses, Registrations: -Graduate of a state approved nursing
assistant training program and passed a state approved competency
examination (written and skills) within 120 days of employment;
name entered into state nurse aide registry.LANGUAGE SKILLS: -Must
be able to express self adequately in written and oral
communication and to communicate effectively in an
interdisciplinary care setting with Residents, families and staff
members.MATHEMATICAL SKILLS: -Ability to add and subtract three
digit numbers, calculate percentage of whole, measure in cubic
centimeters (cc) and measure in feet and inches. Ability to perform
these operations using units of weight measurement, volume, length
and height.REASONING ABILITY: -Ability to apply common sense
understanding to carry out instructions furnished in written, oral
or diagram form.ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: -To
perform this job successfully, an individual must be able to

key functions.C.N.A. Care and Services:
- Accurately perform and record vital signs, height and
weight.
- Accurately calculate and record weight gain/loss and fluid
intake/output.
- Maintain a clean and safe resident environment.
- Recognize abnormal changes in body functioning and the
importance of reporting such changes to a supervisor.
- Perform palliative and post-mortem care.
- Perform routine care rounds on each assigned resident, at
minimum, every two hours (or more frequently as indicated).
- Supervise, assist or provide ADL's such as:
- Bath, shower, whirlpool, and bed-bath
- Shampoo, grooming, shaving, nail care and mouth care
- Dressing/undressing, application of splints/prosthetic
devices
- Toileting, bowel/bladder training, incontinent care an
peri-care
- Meals/snacks, hydration, proper use of feeding
devices/techniques
- AM and PM care per facility policy
- Accurately calculate and record percentage of meal intake.
- Provide preventative skin care techniques, observe the
resident's skin condition and report any changes to a licensed
nurse.
- Utilize proper techniques and devices to transfer and
position/re-position a resident.
- Demonstrate ability to manage and respond appropriately to
resident behaviors.
- Meet the residents' mental health and social needs.
- Is aware of developmental tasks and physiological changes
associated with the aging process.
- Maintain/support the resident's right to maintain personal
choices.
- Able to work effectively with the residents' families and use
them as a source of emotional support.
- Identifies a resident by the use of the identification
band/photo prior to providing care, serving meals, etc., as
necessary.
- Approach all residents in a kind, gentle and friendly manner.
Respect for the resident's dignity and privacy is consistently
provided. Maintain the confidentiality of all resident
information.
- All grievances/complaints made by the resident are promptly
reported to a Licensed Nurse.
- Provide assistance with all admissions/transfers/discharges.
- Room is clean and prepared for a resident prior to
admission
- Resident belongings are labeled, inventoried and stored
properly
- Room is cleaned after a resident is
transferred/discharged.
- Assist with resident room changes.
- Assist with transporting specimens to the appropriate
areas.
- Unit/hallways are cleared of equipment or on one side.
- The Nursing Unit is maintained in a clean/orderly fashion.
- Resident call lights are answered promptly and appropriate
responses to requests are provided.
- Provide nursing rehabilitation/restorative nursing by:
- Promoting the resident's ability to adapt/adjust to living as
independently as possible.
- Training the resident in self-care activities according to the
resident's abilities.
- Training the Resident in the use of assistive devices for
transferring, walking and mobility, dressing and grooming, eating
and swallowing.
- Providing/assisting with range of motion; proper
turning/positioning in bed and chair.
- Providing bladder/bowel retraining and toileting programs.
- Instructing the resident on the care and use of
prosthetic/orthotic devices.
- Incorporating training and skill practice activities that
include repetition, physical or verbal cueing and task
segmentation.
- Provide care for residents with complex medical or nursing
needs, such as those who are:
- Confused/cognitively impaired or Alzheimer's and related
dementias.
- Dependent upon medical equipment such as infusion therapy,
eternal feeding pumps, ventilators and monitors (if
applicable).
- Recuperating from surgery.
- Receiving intensive rehabilitation therapy.
- Neurological residents.
- Cardiovascular residents.
- Respiratory care residents.
- Oncology residents
- Residents with chronic wounds.
- Provide other Nursing Unit tasks including, but not limited to:
- Wiping off wheelchairs, recliners, and positioning
devices.
- Clean/organize clean/soiled utility rooms.
- Cleaning shower and whirlpool rooms.
- Clean/organize nourishment kitchen areas.
- Promptly report all accidents/incidents unusual events during
the shift it occurs.Resident Rights: -Promotes and protects
residents' rights, assists Residents to make informed decisions,
treats Residents with dignity and respect, protects Residents'
personal belongings, reports suspected abuse or neglect, avoids the
need for physical restraints in accordance with current
professional standards and, supports independent expression, choice

- All equipment is operated in a safe manner and the only
equipment utilized that which previous training of use has
occurred.
- The resident's environment is safe and clean.Infection
Prevention: -Applies hand washing principles during daily work,
demonstrates understanding of isolation precautions, recognizes
signs and symptoms of infection and complies with the employee
health program, demonstrates understanding of the process for
identifying and handling infectious waste, maintains personal
hygiene, complies with OSHA standards in the work place and
demonstrates understanding of cross examination.Information
Management: -Enters or records data timely and accurately, protects
confidentiality of Resident information, protects data against loss
or destruction and reports suspected violation of
security/confidentiality issues.INTERPERSONAL SKILLS: -A Certified
Nursing Assistant demonstrates active listening techniques, gains
support through effective relationships, treats others with dignity
and respect and seeks feedback.CONTINUING EDUCATION: -Attends
in-service and education programs and attends continuing education
required for maintenance of professional certification or
licensure.PHYSICAL DEMANDS: -The physical demands described here

full body sling on a resident and operate the equipment during the

- Staff Meetings are attended, if on duty. Minutes are read and

- Follow work schedules and complete assigned tasks according to
the established policies and procedures of the facility.
- Adhere to attendance and tardiness policies.
- Confidentiality of all residents is respected at all times
(i.e., when answering telephone and/or speaking to
co-workers).
- A positive working relationship with residents, visitors and
