Lutheran Social Services of Wisconsin and Upper Michigan has an opening for a unique dual position that includes duties as a Human Service professional at Spring City Corner Clubhouse and a Rehabilitation Specialist in partnership with Waukesha County.
80 percent of the role will directly work with our clients at Spring City Corner Clubhouse. We are a day drop in recovery center for adults suffering from Mental Health challenges. Our goal is to help members experience social contact and develop a structured meaningful daily routine. Duties include, resume workshopping , education and employment assistance.
As a Rehab Specialist, will work in CCS county services.
The Rehabilitation Specialist provides enrichment and work activities for program participants that will aid in their rehabilitation
Will provide Comprehensive Community Services (CCS) at clubhouse and throughout county if applicable
Will attend a weekly CCS supervision meeting with Waukesha County.
This is a full time, Benefit Eligible Role . Monday through Friday
Under direction, perform work involving the delivery of professional human services to individuals, families, and/or groups. This work may be completed in a variety of settings (e.g. home, school, office, community) utilizing various forms of service delivery (e.g. in-person, audio, visual) per the service plan. Performs other duties as required.
At Lutheran Social Services of WI and Upper MI, the health and safety of our colleagues and clients is our top priority! LSS reviews and follows CDC and other public health guidelines. Dependent on program, LSS employees are required to mask based COVID-19 levels. All employees are expected to adhere to LSS health and safety protocols.
Examples of services Expected may include (These are examples, not all inclusive, and all services may not be provided by an individual employee.):

  • Individual Skill Development and Enhancement (e.g. daily living skills - cleaning, hygiene, mentoring of youth, teaching of parenting skills, community engagement)
  • Individual and/or Family Psychoeducation (e.g. mental health, substance use, parenting)
  • Medication Monitoring - Non-Prescriber (e.g. delivery medications to improve compliance, review and teach clients about the medications/potential side effects)
  • Employment Related Skill Training - address the person's illness or symptom-related problems in finding, securing, and keeping a job. Services may include but are not limited to: employment and education assessments; assistance in accessing or participating in educational and employment- related services; education about appropriate job-related behaviors; assistance with job preparation activities such as personal hygiene, clothing, and transportation; on-site employment evaluation and feedback sessions to identify and manage work related symptoms; assistance with work-related crises; and individual therapeutic support.
  • Participation on recovery teams. (provide feedback around service delivery and progress)
  • Crisis assessment and stabilization services - Skill development to reduce or eliminate an individual's symptoms of mental illness so that the person does not need hospitalization. Assist in the transition to a less restrictive placement or living arrangement when the crisis has passed.

      DIRECT SERVICE REQUIREMENTS (Required of all employees providing direct services):

      • Promote client independence and growth, consistent with the service plan.
      • Provide services using a trauma-informed and person and/or family-centered model.
      • Provide services utilizing agency approved and supported evidence-based practices that have been indicated by the funder and/or agency.
      • Accurately complete time reporting.

        Essential Duties and responsibilities:

        • Provide services to assigned clients in alignment with the service plan.
        • Organize and plan work effectively.
        • Apply knowledge of community resources.
        • Apply knowledge of counseling and casework principles and methods.
        • Apply working knowledge of human behavior, dynamics of groups, interpersonal relations, and social interactions.
        • Apply working knowledge of social problems and their effects on individuals.
        • Work collaboratively with clients, service teams, and clinical supervisor to determine interventions.
        • Coordinate services per the service plan.
        • Establish and maintain effective and collaborative working relationships.
        • Complete documentation within required timelines and expectations per LSS, regulation, funders, and practice standards.
        • Maintain client records.
        • Make recommendations within the client team and scope of practice.
        • Testify at legal proceedings as required by law and/or role.
        • Attend client/recovery team meetings.
        • When supervision is a program requirement, actively engage in the clinical supervision process.
        • Attend and participate in staff development programs, including in-service training, staff meetings, and professional seminars.
        • Within your scope of practice, provide consultation to peers.
        • Work a flexible weekly schedule that includes some evening and/or weekend hours for client appointments. Work directly with Program Supervisor or Manager to set expectations for scheduling clients. Schedule is based on client need.
        • Understand productivity expectations. Meet or exceed the standards that have been established for the role and the program.
        • Perform other duties and special projects as required.
