• Provide comprehensive physical therapy services to patients in their homes, including assessment, evaluation, and treatment planning.
• Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
• Develop and implement individualized care plans that address the unique needs and goals of each patient.
• Collaborate with other healthcare professionals to ensure comprehensive care and support.
• Document patient progress and outcomes in a timely and accurate manner.Requirements
  • Current, unencumbered State Physical Therapy License.
  • One year of experience as a home care professional Physical Therapist, with a strong background in comprehensive assessment and OASIS-C.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to make sound professional clinical judgment and prioritize patient needs.
  • Proficiency in Microsoft Office and electronic medical records systems.What We Offer
