Job Description:

Seeking a skilled Registered Nurse to provide in-home care to patients, develop care plans, and coordinate with physicians and caregivers.

The Salaried Home Health Admissions Registered Nurse is responsible for delivering high-quality patient care in accordance with developed care plans and physicians' orders. This role involves admissions for the Brookfield branch.

• Develop a comprehensive care plan through physician orders, client input, and nursing assessment, including medical interventions and measurable goals and outcomes.
  
• Provide education to clients and their family members based on individual needs.
  
• Orient and train primary caregivers to ensure optimal functioning levels for each client.
  
• Coordinate hands-on client care through documentation and timely communication with physicians and caregivers.
  
• Deliver in-home care using various skills, including phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.

      Requirements:
      
      
      • 
        
      • An active RN License in the state of application.
        
      • Valid CPR.
        
        
        Preferred qualifications:
        
        
        • 
          
        • Medicare Skilled Nursing experience.
          
        • Basic understanding of Oasis.
          
        • 1-year RN experience in a healthcare setting.
