Job Description:
Seeking a licensed Physical Therapist for home health services
in Tulare County and surrounding areas, offering full-time or
part-time roles with mileage reimbursement and paid driving
time.
Provide one-on-one, in-home physical therapy tailored to patients
with diverse diagnoses, including orthopedic, respiratory, and
cardiac conditions.

Requirements: Baccalaureate or master--------s degree in physical
therapy, valid state license, at least two years of experience
(home health preferred), CPR certification, and a licensed, insured

Responsibilities involve conducting evaluations, developing care
plans, educating patients and caregivers, documenting progress,
participating in discharge planning, and providing supervision to
assistants.
Engage in interdisciplinary teamwork, quality improvement
activities, and in-service education as needed.
