Join our multi-disciplinary team to provide recovery-oriented services to clients using evidence-based practices. You'll support clients in achieving psychiatric stability through crisis intervention, therapy, medication training, and case management services in the community.
Key Responsibilities:

• Conduct assessments (IM-CANs) to determine service needs and provide referrals and linkage to resources.
• Collaborate with the CST team to facilitate individual & group skill-building activities for community living and recovery goals.
• Maintain clinical documentation, including electronic records and progress notes, ensuring compliance.
• Assist clients in securing entitlements and therapeutic recreational opportunities.
• Participate in on-call rotations and handle crisis interventions as needed.
• Keep families, guardians, and staff informed of client progress and any major changes.Additional Duties:
  • Attend case staffings, clinical meetings, and trainings to stay current with trends and best practices.

    Education, Skills & Experience:
    • Master's degree in Social Work, Counseling, Psychology, or related field (Bachelor's with extensive mental health experience considered).
    • Experience working with clients dealing with substance abuse, severe mental/physical illness, and/or vocational rehabilitation.
    • Must be able to work with diverse populations and manage multi-faceted needs.
    • Strong written & oral communication, decision-making, problem-solving, organizational, and computer skills.
    • Ability to engage clients and work both independently and as part of a team.
    • Valid driver's license and insured vehicle required for travel to multiple locations.
