Contract Duration: 13 Weeks Start Date: 1/26/2026 About the
Position Job Title: RN - Case Manager Location (On-site, Remote, or
Hybrid?): Waukesha, WI (onsite) Contract Duration: 13 weeks Work
Hours: 8AM - 4:30PM Total Weekly Hours: 40 Case Manager RN - Day
shifts (This is an all inclusive rate. Agency would be responsible
for building in mileage into traveler's pay package.) Contract
Length: 13 week Start Date: 1/26 Guaranteed Hours Per Week: 40
hours per week Primary Location: Waukesha, WI Shift Requirements:
Mon-Fri 8hr Days; 8-430pm Travel Requirements: 6 patient visits per
day; approximately spending 1 hour per patient including drive
time, patient visit and charting- 6 county service area- up to 250
miles per week- average 50 miles per day On Call/Call Back
Requirements: none Required Certifications to be Attached upon
Submittal: AHA BLS, Active WI RN or Compact license; Valid Drivers
License - OASIS Cert a PLUS Minimum Experience: Current Home Health
Experience REQUIRED; Oasis experience REQUIRED Charting Experience
Required: Epic required within the past year Unit Information (Bed
size, Case type, Patient ratio): 1:1 patient ratio; adults 18yrs
and older, chronic disease management including CHF, COPD,
Diabetes, Cancer, CVA, wound care, IV administration; Would carry a
caseload of patients and perform all OASIS time points Hard worker,
Time management, Resilient and Manages Change well, Social
candidates Scrub Colors: Any or Corporate Casual Dress Covid-19
Vaccine Required unless Medical or Religious Exemption applies GSA
hours are not guaranteed the week of Orientation. Traveler is
guaranteed set hours per week, except during holidays where unit is
closed Float: Employee should not be asked to work outside of
his/her Clinical skills unless approved by employee, unit manager,
shift advisor, and SimpliFi. Candidate will be required to float to
units and facilities based on census.
