Alliance Services, Inc. is seeking a travel nurse RN Case Management for a travel nursing job in Waukesha, Wisconsin. Job Description & Requirements Specialty: Case Management Discipline: RN Start Date: 01/26/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Estimated Gross Weekly Pay: $ 2560.00 Opportunity: RN - Case Management Job Details: • Work Monday through Friday, 8-hour day shifts from 8:00 AM to 4:30 PM • 40 guaranteed hours per week • 13-week contract duration • Conduct 6 patient visits per day with approximately 1 hour per patient including drive time, patient visit and charting • Travel requirements include up to 250 miles per week (average 50 miles per day) across a 6-county service area • Carry a caseload of patients and perform all OASIS time points • No on-call or call back requirements • Current home health experience required with OASIS experience required • Epic charting experience required within the past year • Patient population includes adults 18 years and older with chronic disease management including CHF, COPD, Diabetes, Cancer, CVA, wound care, and IV administration • 1:1 patient ratio • Required certifications: AHA BLS, Active WI RN or Compact license, Valid Driver's License (OASIS certification a plus) • Associate degree or certificate required • Any scrub colors acceptable or corporate casual dress • May be required to float to units and facilities based on census • Holiday rotation if needed • Manager phone interview required Facility: This home health care organization provides comprehensive healthcare services to patients in their homes across multiple counties.
