Job Description Job Description Prairie Ridge Health is looking
for a team member to join our Patient Care Services Department in
the role of Social Worker. This position is 0.9 FTE (36 hours per
week). This person will be responsible for working with
patients/families, physicians and staff to find resources and
alternative to support the continuum of care post hospitalization
POSITION SUMMARY Effective in working with patients and families,
physicians and staff to find resources and alternatives to support
the continuum of care post hospitalization. Demonstrates analytical
skills to understand and interpret complex behavior and obtain
appropriate clinical consultations or clinical referrals for post
discharge. Provides leadership and support for the hospital Ethics
Committee. Is the patient advocate for advance directive planning
and end of life/palliative care options, both for our inpatients
and within the communities we serve. Serves as an educational
resource for the community regarding different options and
resources available, such as Swing Bed, Respite, etc. Position in
City Emergency Preparedness Plan. EDUCATION
REQUIREMENTS/LICENSURE/CERTIFICATION/REGISTRATION Current WI
license as a Social Worker required Experience in acute and long
term care delivery preferred Computer experience preferred FTE: 0.9
Shift: Monday-Friday, days
