Our highly skilled nurses provide a wide array of services,
including medication education and management, injections and
intravenous therapy, wound care, nutritional counseling, bowel and
bladder management and much more. Physical, occupational and speech
therapists address home safety concerns, strengthening and
rehabilitation needs. Our licensed social workers offer assistance
with community resources, financial assistance and supportive care
and counseling to patients and families.

Schedule: Work 2200 to 0800 every weekend (Fri. / Sat. / Sun.)
with one weekend off per quarter. Holidays are worked if they fall
within the static schedule Commit to work at least 3 out of 4
weekends per month Will work the holidays that fall on the weekend
or will work scheduled holidays per department rotation schedule.
Additional Pay: Weekend Night Shift (7:00 PM - 7:30 AM) - $7.00 per
hour + $2.80 Night Differential
What You Will Do:
The Triage and Visit Nurse (internally titled RN - Home Hospice)
in Home Care and Home Hospice works collaboratively with patients,
nurses, social workers, physicians, other practitioners, caregivers
and community agencies in coordinating and overseeing the
interdisciplinary plan of care for a patient in a home care/home
This nurse may develop the plan of care, follow an established
plan of care, or make modifications based on their assessment of
the patient. This nurse for home care & home hospice is responsible
for improving the health of patients dwelling in their own homes.
This requires a nurse who is comfortable working independently,
enjoys teaching, is a critical thinker and has strong communication
skills. They provide hands on care in the home environment and are
offered autonomy, flexibility and the luxury of caring for one
patient at a time.
What you will need:
- Associate's Degree in Nursing required. Bachelor's Degree in
- Current State of WI or eNLC compact state RN license.
- Valid State of WI driver's license with an acceptable driving
- Current BLS (CPR) certification or ability to obtain upon
- Clinical experience in home health preferred.
- EPIC software experience preferred. *CA #LI-MO
